We’re not a national lead aggregator or call center. We’re a locally rooted, hands-on placement agency that focuses exclusively on small to mid-sized communities (under 100 beds)—the kind of places where care is personal, residents are known by name, and dignity is a priority.
Founded by Silvia and John Caravella, Elevate was born from 14+ years of experience in staffing and placement. While planning the launch of our own 15-bed residential care home, we saw how overwhelming this process is for families—and how broken the referral system can be.
So we built something better.
We guide every client personally, from the first phone call to move-in day—offering honest insight, clear options, and unwavering support. Whether you're looking for assisted living, memory care, or a residential-style group home, we’ll help you find the right fit—with care, compassion, and clarity.

We handpick 2–3 care options that best align with your priorities, including assisted living, memory care, and small group homes.

Tours & Coordination
We arrange and coordinate tours, provide key questions to ask, and can join you—virtually or in person—if you’d like extra support.
